We are always happy to help and support our patients during their stay in our polyclinic.

This document details our terms and conditions of service. Please note that any information provided on our online platforms (examples: website, forum, blog, social networks, telephone, email, etc.) is intended for information purposes only. and does not in any way constitute personalized professional advice. Indeed, professional advice can only be given during a clinical consultation with our doctors and surgeons. .

Article 1 – Treatment planning

Once your treatment plan has been agreed with one of our GPs and specialists, we will provide a written estimate of your plan. This plan may change due to other exams additional radiographic or clinical information. If this is the case, we will inform you and discuss with you accordingly. Treatment plans are valid for 30 days from the date of prescription date. Any questions about your treatment plan should be asked before start treatment.

Article 2 – Consent form

Some treatments require a written consent form to ensure that you have understand the treatment, care and risks in depth before it is carried out. This form also allows you to have clear information and to take a objective decision before starting the treatment in question.

Article 3 – Late cancellation or missed appointment

Clinique Plaza reserves the right to charge an amount in the event of omission or cancellation of an appointment with less than 24 hours notice. For interventions of more 1 hour, we need at least 72 hours.

Additional waits may occur in the waiting room due to an emergency or other medical constraints.

Article 4 – Personal information

It is very important that you provide a complete medical history and inform your treating physician of any medical changes.

Article 5 – Use of images

Clinique Plaza  may use images and x-rays for purposes promotional, educational and scientific. Your name or identity will never be revealed or published. However, if you do not want your images to be used, please let us know.

Article 6 – Use of patient contact details

Your health is our top priority. To do this, we would like to inform our patients of the various changes that have occurred at the clinic or to remind them of their appointments. Indeed, the clinic can contact you periodically by telephone, SMS, email or postal mail.

Article 7 – Complaint policy

We take any complaints relating to one or more aspects of our services seriously, in order to ensure that every patient can have an excellent experience at our clinic. YOU can submit your complaint by completing a simple evaluation form available at the reception. Complaints must be clear so that they can be handled effectively.

We will endeavor to resolve the problem as quickly as possible. If a complaint concerns a aspect of clinical care, it will be addressed directly to the attending physician.

Clinique Plaza reserves the right to cancel, modify or postpone the treatment plan of a patient in the event of non-compliance with these conditions by the latter. All possible reimbursement will be subject to a deduction from the contributions for the work already carried out on the patient.

Article 8 – Protection of personnel

Abuse of doctors and staff, loud/disorderly/drunk behavior, Late cancellations of appointments (repeated) are in no way tolerated. In these situations, the  Clinique Plaza reserves the right to refuse treatment to the person in question.

Article 9 – Data protection

Our patients’ personal data is stored on a secure computer system. in accordance with data protection law. All clinical notes, radiographs or digital photographs remain the property of La Clinique Plaza. Copies can be made available upon request. Clinique Plaza reserves the right to invoice administration costs for these operations.
